QUARTERLY PROJECT TRACKING


   

MACOG requires that each federally funded project submit three (3) forms each quarter.  The forms are:  Quarterly Tracking Form, Milestone Sheet, and Financial Plan.  The forms are available on-line (below) or by calling MACOG at (574) 674-8894.  If you have additional questions, please call MACOG.

Quarterly Project Tracking will occur at every fourth MACOG Transportation Technical Advisory Committee (TTAC) meeting.  Tracking is a way to make sure the LPAs, their consultants, and INDOT are all on the same page.  Quarterly Tracking Forms are due to each quarter (October, January, April, & July).  Each LPA should send their Quarterly Tracking Forms (QTFs) to MACOG via the FTP site, approximately two weeks before the Quarterly Tracking Meeting.  All TTAC meetings are at 9:00 a.m. and located on the 11th floor of the County-City Building, 227 W Jefferson Blvd, South Bend, IN.  More specific instructions will be e-mailed to LPAs & consultants before each meeting.  

QTFs are due to MACOG by December 19, 2011 and the next Quarterly Tracking Review meeting will be held January 4, 2011 immediately after the TTAC Meeting.  The QTFs are due to INDOT by January 21, 2011, however MACOG will submit your forms to their respective INDOT Districts before this date.


Quarterly Tracking Forms

Below are the three (3) forms you need to submit for each federal aid project.  For each project, the consultant needs to submit (1) Quarterly Project Update Form, (2) Milestone Sheet, and (3) a Financial Plan.  Both the Quarterly Tracking Update Form and the Milestone Sheet need to be submitted as a word document (.doc).  The financial plan can be submitted as an excel or pdf file format.
